The following enrollment forms are required for all Greyford Academy applicants. Please be sure to complete the forms in their entirety. Each form has a comprehensive explanation of its purpose in the admission scheme beside it. Thank you for your interest in Greyford Academy. We are excited to get you and your little one registered and nestled in.

A Passport Sized Photo of the child
A Copy of the photo page of the child's passport or a copy of his/her birth certificate
A Copy of the child's immunization records
A Completed Greyford Academy Enrollment Application ( found below)
A Completed Minor Release Form (found bellow)
A Completed Medical Form (found below)
A Completed Insurance Form (found below)
A Completed Get to Know My Baby Form (found on our website)
A Negative Covid Antigen Test (dated no more than 5 days pre the child's first day on premises)
A Completed Covid19 Declaration Form (found below)
Proof of payment: Non Refundable Seat Fee
Proof of payment: Non Refundable Registration Fee
Proof of payment: Term Tuition
At least 1 set of uniforms for the child.

Applications submitted without all of the above will not be processed.
Partial Applications cannot be processed. Please ensure that your child's forms are completed in its entirety and signed.

Students entering Greyford Academy at Pre-Kindergarten & Beyond are required to sit an Assessment Test at a cost of $250.00 before placement is made.

It is important to remember that Greyford classes are all filled on
a first pay basis, seats are not held that aren't paid for.
​
This form, kept on file during your child's education here at Greyford is used to gather facts about the child, parent and previous education to allow the school a comprehensive overview of where the child should be placed and detailed information like who should be picking up etc.​
This form, has two main parts, a portion for completion by the parent/guardian and one part that should be completed solely by the child's physician. It allows the school a comprehensive look at the child's medical history to determine if there are things we should be looking for while he or she is in our care and also serves as a reference point should emergencies arise.
BAF Financial provides insurance coverage for children while in our care, while on campus. The insurance is paid for annually to ensure that if something happens to your little one, your child is taken care of. Find coverage inclusions and exclusions here.

The Covid19 Declaration Form is a requirement by The Bahamas Preschool & Daycare Council.
It provides a series of questions that once answered are a declaration of your child being free from Covid19 based on a collection of symptoms and to the best of your knowlege.
The Get To Know My Baby Form is designed to allow Mom and Dad an opportunity to explain the unique habits, preferences and dislikes of their child to better enable his or her care taker to
offer the absolute best care in the absence of the parent.
